Reports To:  Team Coordinator

Position Summary:

Under the guidance of a Team Coordinator and through active team participation, the Support Worker (2) will be responsible for providing assistance, support and direct supervision of adults with developmental, physical and emotional challenges. The Support Worker (2) will provide these individuals with creative opportunities that facilitate learning, participation and skill development in their community.

Qualifications:

  • Experience in working with adults who exhibit multiple challenges in a community or center based setting
  • Posses demonstrated abilities in a designated field of specialty as indicated by the needs of the assigned individuals
  • Will possess a valid driver’s license

Responsibilities and Duties:

Working effectively with a Team(s), a Support Worker (2) will undertake the following duties and responsibilities:

Person Centered Planning and Support:

  • Will meet the basic physical care and personal needs of individuals
  • Complete shift duties as assigned
  • Will be familiar with and follow medication procedures as outlined by the agency’s policies/procedure manual
  • As identified through personal plans, will develop and implement activities suitable to allow individuals to participate effectively in the community.  May include but not be limited to the following general areas:
    • Education
    • Computer literacy/office systems
    • Voluntary community activities
    • Augmentative communication
    • Recreation
    • Daily independent living skills
    • Competitive employment
  • Ensure all activities are in accordance with the principles and philosophy of Community Living Guelph Wellington
  • Provide transportation to individuals via association, public transit or personal vehicles as required
  • Communicate with sensitivity and respect with all people to enhance the dignity and quality of life of others

Team Functions:

  • Actively participate in meetings, discussions and decision making
  • Conduct themselves in a positive and professional manner at all times
  • Must be willing to work flexible hours and within a variety of settings as required
  • Will understand and utilize formal and informal methods of communication
  • Will possess effective interpersonal and problem solving skills
  • Will be a self starter
  • Will keep regular and accurate records and documentation
  • Provide guidance and orientation to new members of the team including volunteers

Organizationally:

  • Understand and carry out policy and procedures of the association
  • Perform any other associated duties as specified by the Team Coordinator and/or Service Manager

Health and Safety:

  • Ensure that Health and Safety, Serious Incidents/Occurrences and Crisis Management Plans are understood, followed and reported
  • Will possess and demonstrate knowledge of First Aid, CPR, NVCPI, WHMIS, Occupational Health and Safety Act, fire prevention and safety procedures and other areas as deemed necessary by the association.
  • When required, must be able to undertake lifting of people following guidelines for proper back care, lifting and transfers (knowledge of manual and mechanical lifting techniques)
  • Immediately advise the team coordinator or Health and Safety Rep of issues/problems/concerns regarding participants, staff, buildings, equipment and vehicles

Hours of Work:

  • Relief – Hours will be offered on an ad hoc basis based on needs.  Requires evenings, night shifts and includes weekends and holidays as required. Will not normally exceed 40 hours per week.
