Person Centred Approach

Community Living Guelph Wellington is committed to a person centred approach to providing services and supports to people.

• Planning is based on the needs and desires identified by the person being supported and may include family and/or friends.
• Supports provided must be adequate and appropriate.
• Each person receiving support will have a support plan or lifeplan document that outlines the unique and individual supports and services they desire.
• If a person feels a lifeplan is not appropriate or desirable this will be respected, unless clgw is unable to deliver appropriate supports with a Support Plan or Lifeplan
